A pilot study in UK by Gray and the group discusses the fluid choice in sepsis resuscitation. Albumin or balanced crystalloid? So what's new? This study aptly named ABC sepsis trial takes this question to the emergency department and provides some key insights into the very initial fluid choices during the early hours of sepsis, when you're still thinking about ABCs It also provides some very interesting findings in sepsis subgroups. Fluid resuscitation ninjas, you gotta check this study out. The second study is for my ultrasound and neuro ICU folks. An international consensus statement on Imaging and Measurement of Optic Nerve Sheath Diameter. This Delphi consensus from 52 global experts describes technical details and a checklist for ONSD measurements. A thoughtful editorial by Paul Nyquist outlines pearls and pitfalls of looking for shadows in the black box. Lastly, Wang and the group report on quality of methodology and recommendations across 14 liver failure guidelines from diverse sources. This review article helps provide a framework for critical review during guideline updates and highlights a research gap in evaluating the impact of such clinical recommendations.
