Nnenna Nwanonyiri: Hi, my name is Nnenna and as an RN case manager at Whole Health by Quartet, I support our hospitalized patients or patients in skilled nursing facilities during their transitions of care events with important discharge needs like medication management, making doctor's appointments, and addressing social determinants of health. This is accomplished through care coordination involving multiple disciplines and members of our Whole Health team and other outpatient organizations and services. Coordinating care can be an overwhelming process for a patient recently hospitalized or in a skilled nursing facility. I am grateful for the opportunity to be a resource and help make that process easier. My work helps patients avoid rehospitalization, achieve their goals, manage their conditions, and live their fullest lives. And that is why we care.
